Studio Policy
NO SHOW/ LATE CANCEL/ FAILURE TO PAY POLICY
Any cancellations with less than 24 hours of notice are subject to a cancelation fee amounting to the total cost of the scheduled service. Clients who miss their appointments without giving any prior notification will be charged in full for the scheduled service. We recognize the time of our clients and staff is valuable and have implemented this policy for this reason. When you miss an appointment with us, we not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time. Additionally, many times our staff will be functioning in an "on call" status and can have travelled specifically for your service. For these reasons we are obligated to compensate our staff for their time as well as make up for the lost revenue.
CANCELLATION/NO SHOW/ RESCHEDULE POLICY
Please provide at least 24 hours notice if you need to reschedule, cancel an appointment, or reduce your appointment hours. This gives us enough time to fill this time slot.
If you call with less than 24 hours notice, you will be charged the full cost of the massage unless you reschedule your appointment immediately. If you reschedule immediately you will only be charged a $50 fee no exceptions (including cancellations due to emergency situations or Covid 19). All missed appointment fees follow our standard 24 hours to pay or they will be subject to the late fee charge
If you call with 24-48 hours notice and you do reschedule immediately, the full cost charge of the massage will be waived, however the $50 cancellation fee will NOT be waived and must be paid within 24 hours to avoid a late fee
Cancellations or appointment modifications made by our returning and new clients who do not reschedule their services with less than 24 hours notice may be charged the total cost of the scheduled appointment. No refunds will be provided for deposits or cancellations received less than 24 hours before a scheduled appointment.
All Clients new to Lakeside Massage and Wellness Studio will be required to pre pay for the full cost of the massage as a deposit to pre book an appointment
If you fail to cancel 24 hours prior to your scheduled appointment slot, and/or you do not show up for your appointment, you will be charged full price. Full refunds will not be given except in emergency situations and appropriate documentation will be requested prior to waving the fees.
LATE ARRIVAL POLICY
We regret that late arrivals will not receive an extension of scheduled onsite massage appointments. In special cases, and when our schedule will allow, we may be able to accommodate a partial or full appointment. This will be at our discretion and only with proper, advanced notification of your late arrival. The original reservation fee will be charged.
Inclement Weather and Emergency Cancellation Policy
If an appointment is cancelled by the client or therapist for inclement weather, emergency or other valid reason (accident, covid-19 shutdown etc.), no fee will be charged to the client and we will rescheduled for a future date when it is safe to do so. Lakeside Massage and Wellness Studio reserves the right to cancel if it jeopardizes the safety of our therapists.
COVID-19 APPOINTMENT CANCELLATIONS must be cancelled prior to the appointment start time unless it's an emergency room situation. A $50 fee will be charged for canceling.
NO SHOW POLICY
Clients who fail to show for onsite massage appointments will not be given a full or partial refund. It is important that our onsite massage therapists are compensated for reserved time slots.
INFORMED CONSENT
Prior to each onsite massage session, the treatment plan will be discussed with you. At your first visit with us you will receive an electronic copy of the onsite massage therapy policies (if you haven't already filled it out online) and will be asked to electronically sign the consent stating that you have read the information, understand it, and agree to comply with the professional onsite massage therapy policies and procedures. This will also have been emailed to you to fill out prior to your first visit.
CLIENT PAYMENT POLICIES
Payments for services are due at the end of the day on the day of service
A valid credit card can be used to pre book your massage services
All new online Clients will be required to place the full cost of the massage as a deposit to pre book an appointment
If paying by check, checks must be received 48 hours prior to your scheduled appointment in order to receive massage services.
If paying by ACH draft, draft must be received 48 hours prior to your scheduled massage services.
Late payments are subject to a $50 late fee after the first 24 hours. If your payment is still late 30 days after the initial late fee, a $100 late fee will be added at 12:00 AM, and an automatic $50 fee will be added to the late balance for every 30 days thereafter. These late fees are your responsibility to pay and will not be reversed.
Fee for returned checks will be a $100 additional fee applied to your outstanding invoice.
UNPAID PIP, 3rd PARTY & INSURANCE CLAIMS
Unpaid PIP, 3rd Party and all other Insurance Claims are your responsibility. Unpaid Massage will be due immediately at the time of service or when it is found that your coverage has been exhausted. Late payments will follow our standard $50 late fee after 24 hours and then a $50 dollar fee for every 30 days thereafter until your account has been paid in full. You do have an option to pay $200 a month towards your balance due: In doing so, the $50 per month fee will be waived but not the initial $50 (1 time) late fee.
If we a working with your lawyers, a 90 day extension can be made on a case by case basis and late fees will not start to accumulate until the 91st day. At 12:01 am on the 91st day, a 1 time $100 fee will be charged for each bill that has not been paid and $50 dollar fees on each unpaid bill will accumulate each month thereafter until your balance is paid in full.
We require you to place a valid credit card on file or late fees will be automatically assessed if the card does not process. This goes for regular payment processing as well as unpaid insurance balances due
